WHO WE ARE

Dyslexia Services provides a service of diagnostic assessment for Dyslexia, tailored one-to-one tuition and diagnostic interview for Attention Deficit Hyperactivity Disorder (ADHD) and DevelopmentalCoordination Disorder/Dyspraxia (DCD).  This privacy policy tells you what to expect us to do with your personal information. We set out how we protect your personal information in accordance with the General Data Protection Regulations (GDPR) and in compliance with the 2018 Data Protection Act (DPA 2018). TYPES OF DATA WE COLLECT

The personal information we collect or have about you when we provide tutoring or diagnostic assessment for specific learning difficulties include:

· Personal identifiers and biographical information - such as name, date of birth and gender

· Contact details – such as home address, email address and telephone number

  • Sensitive personal data – such as details of:

    • why you sought an assessment,

    • disability or specific learning difficulty,

    • support you may have had in the past or currently,

    • previous assessments and any resulting written reports

    • developmental milestones,

    • previous learning performance,

    • previous and current health,

    • diagnostic assessment data,

    • notes written during and after learning support or assessment sessions

    • correspondence or other contact with you, parents, carers, teachers, employer

  • Meetings held with you or associated persons

  • Family details – such as information of other family members experiencing similar problems or with whom you or your school/college/workplace have given us permission to liaise with.

HOW WE COLLECT DATA

We collect information through questionnaires, application forms, email and discussion. We communicate primarily via email and telephone and will keep your details in our email system in order to communicate with you.

LAWFUL BASES

Our lawful bases for collecting or using personal information to provide services are consent, legal obligation and legitimate interest: This collated information is a valuable part of the assessment process The information is required to provide evidence of learning experiences and opportunities and how these impact the learning trajectory. The information may also provide insight into how this experience may have been affected by health or environmental factors. Examination of the information may provide evidence of the management or coping strategies used to mitigate past and/or ongoing challenges.

HOW WE STORE YOUR DATA

We store the information we collect about you electronically and/or on paper. Following the assessment and the compilation of the final assessment report all your questionnaires, all raw data test sheets and most of other personal data will be permanently deleted/destroyed. Electronic data is password protected and kept in a secure external electronic drive.

HOW WE SHARE YOUR DATA

We will only share your personal data with nominated persons identified before the assessment takes place. Sharing of data which is password protected, is done via email to allow us to carry out the contracted services. You will be asked to sign a privacy agreement clause before the assessment. This is included in the pre-assessment questionnaire.

HOW LONG WE KEEP YOUR DATA FOR

All personal information collected for assessment purposes will be safely destroyed once you have received and accepted your diagnostic report. Your final assessment report will be held by us for 7 years from the assessment date. During this time you will be able to ask for an electronic copy of the report. Please note there would be an administration charge for this.

We would strongly recommend that you keep a copy of your report securely. After this time your assessment report will be safely destroyed and we will not be able to supply you with a copy of your report.
